PATHOLOGY SERVICES AND CONSULTS / SECOND OPINIONS

1. For routine technical and professional anatomic and clinical pathology services:
   
   To order test requisitions and supplies click here, or contact our Client Services Center at 941.362.8900.


2.  For molecular and reference laboratory tests:
   
   To order complete SaraPath's “Request for Additional Testing” form located to the right.
   
   Note: If ordering a test performed at Foundation Medicine or Genomic Health, these facilities require completion of their specific test order form, also located to the right. 


3. For a pathology consult / second opinion:

For a consult by a pathologist at another facility (on a case previously interpreted by a SaraPath pathologist): Complete the “Send Out Request for Medical Consult” form to the right and SaraPath will send the patient's slides to the consult facility. If requesting a Moffitt consult, the “Moffitt Pathology Consultation Request Form” must also be completed and signed by the physician. Moffitt will not begin the consult if the order is not signed by the physician.

For a consult by SaraPath (on a case previously interpreted by a pathologist at another facility): Complete the "Request for SaraPath Consult" form to the right.

For a SaraPath consult on a rectal cancer case (NAPRC): complete the “Request for SaraPath Services- NAPRC Rectal Cancer” form to the right.

COMPLETING AND SUBMITTING THE ORDER FORMS:

1. Sign the order form authorizing the order and the release of any patient materials. SaraPath is not able to process your request without a signed order form. Also include a contact name and phone number on the order form, in case we need to contact you for additional information about your order.


2. Also provide the patient’s billing and insurance information, as SaraPath does not see patients and our information may be outdated.


3. The SaraPath request forms can be completed and submitted to SaraPath online, by clicking the submit button located on the bottom of each form.  The request forms can also be faxed to SaraPath’s medical records coordinator at 941.362.8944. The Moffitt, Foundation Medicine and Genomic Health order forms can also be faxed to SaraPath.
